In days gone by a talk titled “refractive vitreoretinal surgery” was likely a misnomer, with VR surgery mostly focused on anatomical success over refractive outcomes. However, the art of vitreoretinal surgery has advanced significantly in recent years along with the visual expectations of our patients. In this talk we discuss the complex interplay between various VR conditions and their refractive implications, and vice versa anterior segment/refractive pathology that might impact the vitreous and retina. We review the evidence around these conditions and my personal experience when trying to achieve the best outcomes for both the retina and the refraction.
